			<title>经济转型期的医患冲突和秩序重建</title>

<p><font size="2"><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">随着中国从计划经济向市场经济飞速发展，转型期的医患冲突发展到有史以来最尖锐的状态。清华大学国情研究中心的研究指出：中国卫生服务的不公平性和慢性病发病率的上升，导致了严重的社会矛盾，医患冲突屡见不鲜。近年来，发生了重庆病人腰缠炸药与医护人员同归于尽；广州病人持斧头砍伤医生；武汉</span><span style="FONT-SIZE: 14pt">200</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">多个病人家属冲击医院；东北病人家属在医院大厅摆设灵堂等事件。中国医师协会</span><span style="FONT-SIZE: 14pt">2004</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">年的一项调查显示</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun">，</span><span style="FONT-SIZE: 14pt">74.29%</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">的医生认为行医的合法权益不能得到保障，其中认为&ldquo;较差&rdquo;和&ldquo;极为恶劣&rdquo;分别为</span><span style="FONT-SIZE: 14pt">47.35%</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">和</span><span style="FONT-SIZE: 14pt">13.28%</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">。有许多当医生的父母得知子女考上医学院后，托人把子女从医科转到其它专业去学习。</span><span style="FONT-SIZE: 14pt"></span></font></p>
<p><font size="2"><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">这些都清楚地说明医患冲突已经严重到水火不兼容的地步，若不能尽快解决，势必引发更多、更严重的社会矛盾和危机。英国医学社会学家提出&ldquo;医疗保健和卫生服务是政治哲学的体现&rdquo;。</span><span style="FONT-SIZE: 14pt"> </span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">中央提出发展和谐社会和温家宝总理两会期间提出老百姓&ldquo;看病贵、看病难&rdquo;的问题得到国务院、卫生部等的高度重视，政府相关部门准备推出一系列解决方案，如深化城市医疗服务体制改革、取消医院药品的批零差价、限制明星做药品保健品广告、甚至准备全面取消医疗广告等等，这些无疑对改善医患关系缓和医患冲突起了积极作用。但能真正从根本上解决问题吗？笔者认为，医疗问题并不是一个简单的医学技术问题，而是包括医学、社会学、经济学、管理学、心理学等众多学科在内的综合性社会问题。这个问题看清楚了，解决的就是根本问题。因此要解决转型期的医患冲突不能只依靠卫生部，更需要国务院的支持，才能达到标本兼治的目的</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun">。</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">笔者愿抛砖引玉提出一些个人的见解，供患者、社会大众、医院管理者及相关部门领导者参考。</span><span style="FONT-SIZE: 14pt"></span></font></p>
<p><font size="2"><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">造成转型期医患冲突的因素众多，有生理因素、心理因素也有社会因素。笔者将逐一加以剖析。</span><span style="FONT-SIZE: 14pt"></span></font></p>
<p><font size="2"><span style="FONT-SIZE: 14pt; mso-bidi-font-family: SimSun"><span style="mso-list: Ignore">（一）</span></span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">病人因素：病人怎么啦？！</span><span style="FONT-SIZE: 14pt"></span></font></p>
<p><font size="2"><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">看病没钱是最重要的原因之一。第三次国家卫生服务调查结果显示，由于经济约束，农村和城市分别有</span><span style="FONT-SIZE: 14pt">75.4%</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">和</span><span style="FONT-SIZE: 14pt">56.1%</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">的患者需要住院而未能住院，而且这个情况还有日益恶化的趋势。北京大学中国经济研究中心副主任李玲认为，城市医疗体制改革面临的关键问题是中低收入人群的可及性差，表现为&ldquo;看病贵&rdquo;和&ldquo;一部分病人看不起病&rdquo;。因服务差而未就诊的人次在迅速上升。医疗消费对价格越来越敏感。</span><span style="FONT-SIZE: 14pt"></span></font></p>
<p><font size="2"><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">看病贵和看病难说的是两个问题：&ldquo;看病贵&rdquo;属于经济性问题，是老百姓负担不起的问题。&ldquo;看病难&rdquo;属于可及性问题。</span><span style="FONT-SIZE: 14pt"></span></font></p>
<p><span style="FONT-SIZE: 14pt"><font size="2"><span style="mso-spacerun: yes">&nbsp;</span></font></span></p>
<p><font size="2"><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">我们仔细研究一下这两大问题。</span><span style="FONT-SIZE: 14pt; mso-fareast-font-family: PMingLiU"></span></font></p>
<p><font size="2"><span style="FONT-SIZE: 14pt; mso-bidi-font-family: SimSun"><span style="mso-list: Ignore">（1）<span style="FONT: 7pt 'Times New Roman'"> </span></span></span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">先富起来的一部分有钱人，&ldquo;看病贵&rdquo;不是问题，但&ldquo;看病难&rdquo;仍然是个难题。最近各医院推出高价&ldquo;特需医疗服务&rdquo;，可以满足这部分人的需求。</span><span style="FONT-SIZE: 14pt"></span></font></p>

<p><font size="2"><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">中国社会科学院的于建嵘博士</span><span style="FONT-SIZE: 14pt">2004</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">年跟踪研究</span><span style="FONT-SIZE: 14pt">200</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">多次典型的工人抗争原因，发现</span><span style="FONT-SIZE: 14pt">37.5%</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">是关于国有企业改制，</span><span style="FONT-SIZE: 14pt">23.5%</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">是拖欠工资，</span><span style="FONT-SIZE: 14pt">10.5%</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">是关于社会保障、医疗福利的问题。</span><span style="FONT-SIZE: 14pt"></span></font></p>

<p><font size="2"><span style="FONT-SIZE: 14pt"><span style="mso-spacerun: yes">&nbsp;</span>(3) </span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">广大农村的农民。</span><span style="FONT-SIZE: 14pt"></span></font></p>
<p><font size="2"><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">虽然&ldquo;新农合&rdquo;在部份地区开始试验，但许多地区还没有开展。该计划有赖政府长期拨款维持。农民看病基本上是听天有命，有多少钱看多少病。</span><span style="FONT-SIZE: 14pt"></span></font></p>

<p><font size="2"><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">作为知识分子的医生，承担着救死扶伤的崇高使命。</span><span style="FONT-SIZE: 14pt">SARS</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">来临时医护人员被称为&ldquo;白衣天使&rdquo;，事过境迁，目前获得的社会地位与尊重却越来越低，是什么原因造成这种状态的呢？</span><span style="FONT-SIZE: 14pt; mso-fareast-font-family: PMingLiU"></span></font></p>
<p><font size="2"><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">转型期的医疗经济行为中存在着&ldquo;一国两制&rdquo;的现象，医护人员的无形劳务收费按社会主义计划经济标准，而有形的医疗辅助物资如药品、仪器、设备、耗材的收费则按改革开放的市场经济标准。比如做一个心导管手术，进口涂药支架高达</span><span style="FONT-SIZE: 14pt">3</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">万多元一支，而手术费则只有几百上千元。医疗收费的&ldquo;一国两制&rdquo;导致墙内不够墙外补，洐生出&ldquo;以药养医&rdquo;等弊病。</span><span style="FONT-SIZE: 14pt"></span></font></p>
<p><font size="2"><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">转型期的医疗行为另一方面由于缺乏初级医疗管理制度（</span><span style="FONT-SIZE: 14pt">Primary medical care</span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">）病人可以随意选择医院、医生，而且各级医院收费标准差异不大，导致多数病人为保险起见选择到大医院就医。结果是大医院门庭若市，医生工作量大，超负荷运行造成服务质量降低，对病人的人文关怀不足，也较容易出现医疗差错。而小医院门可罗雀，病人不足，易引致小病大治等问题，甚止出现医院为了提高床位使用率让不必要的病人住院，这是医生诱导的过度医疗。此外，不必要检查的另外一个原因是&ldquo;举证倒置&rdquo;，</span><span style="FONT-SIZE: 14pt"> </span><span style="FONT-SIZE: 14pt; FONT-FAMILY: SimSun; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">为以防万一，医生让病人花钱检查证明医疗没错误。</span><span style="FONT-SIZE: 14pt"></span></font></p>

<p><span style="mso-spacerun: yes">&nbsp;&nbsp;&nbsp; </span><span style="FONT-FAMILY: 宋体; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">艾力彼公司的研究表明，对病人投诉的管理不仅仅是医务科，护理部的事情，只有当医院第一把手高度重视，当医院所有的部门和人员相互支持，配合，共同建立一个良好的病人投诉管理系统，才有可能使病人投诉的问题得到解决，才能使投诉的病人转变为医院忠诚的客户，才能从投诉中挖掘有价值的信息提高医院管理水平，才能使投诉管理从一个负面被动的管理过程转变成一个积极、主动的营销过程，使其成为医院创造价值和树立品牌的过程。</span></p>

<p><span style="FONT-FAMILY: 宋体; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">许多医护人员把投诉当成一个烫手山芋，怕被投诉。有的医院不分青红皂白对被投诉人扣罚奖金、片面追求&ldquo;零投诉&rdquo;等，从而导致医护人员都采取&ldquo;大事化小，小事化了&rdquo;的精神，有投诉能瞒则瞒，能不上报就不上报。但正如前文图</span>1<span style="FONT-FAMILY: 宋体; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">所示，病人中只有</span>5%<span style="FONT-FAMILY: 宋体; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">会正式提出投诉，这意味着每一位投诉的病人代表着</span>20<span style="FONT-FAMILY: 宋体; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">位不满意的病人，投诉者是自告奋勇地代表对医院医疗服务不满意的病人，积极主动地向医院提出工作中存在的问题。如果没有他们的自告奋勇，我们则看不到自己的不足，病人都悄悄地跑到竞争医院去了。同时还要清楚地意识到病人的投诉中有许多是给我们提出的意见和建议，因此，病人投诉可以帮助医院改进和创新服务。投诉能够告诉医院：哪些地方出了问题；哪些是薄弱环节；什么地方没能满足病人的期望；他们甚至会指出竞争对手在哪方面表现的比我们强&hellip;&hellip;。理论上讲，这些信息是需要付咨询费才可能获得的，而现在病人将其&ldquo;免费&rdquo;送给我们，我们应该好好的感谢他们。投诉者应该得到我们的尊重。他们是我们的朋友，是为了我们好，才把信息告诉我们的。病人的投诉信息如果被正确对待和分析处理，将是医院管理非常有价值的资源，投诉信息中可能隐藏着我们容易忽略，但又非常有价值的信息，可帮助医院改善医疗质量，就医流程和服务规范化等诸多方面。</span></p>
<p><span style="FONT-FAMILY: 宋体; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">由上述的分析可以看出，病人投诉对提高医院管理水平是很好意义的。医院应该系统性地看待病人投诉，建立一套投诉管理体系，建立系统纠编和个体纠编机制，确定投诉管理的目标和意义，明确医院内各相关部门人员对投诉承担的职责和权限，建立投诉管理中心，制定一系列投诉处理的工作流程和标准，让投诉管理变成一项医院管理的增值业务，让不满意的病人变成忠实顾客。</span></p>

<p><span style="FONT-FAMILY: 宋体; mso-ascii-font-family: 'Times New Roman'; mso-hansi-font-family: 'Times New Roman'">艾力彼医院管理研究中心连续几年来，一直非常关注医患关系的现状和发展。我们通过对北京、上海、广州、武汉、天津、南京、杭州等国内大城市几十家医院的调研，搜集大量的第一手资料，探索导致医患关系紧张的诸多源因，找寻解决方案。我们认为，导致医患关系紧张的原因主要有如下几点：体制因素、医方因素、患方因素、媒体因素。</span></p>
